1. Buy a ticket in the Saturday Lotto Megadraw (Draw 4639 – 3 Jan 2026).
2. Complete the entry form below if you purchased your ticket in-store. If you've purchased using your Lotterywest membership online, via the app, or in-store, you're automatically entered and do not need to complete this form.

Please note - to claim this prize, you will need to present the physical ticket (unless purchased online), so hold on to it after the draw!
